First use a simple, comprehensive business model with which to compare company processes.  It acts as a lens focused on where people meet process, gradually building the complete story of your business.  It also acts as a framework on which you can hang key definitions and processes on. 

Second, instill a process of structural tension into your organization that supplies the energy, focus and ownership of processes needed to carry out your plan and overcome the structural conflict that usually acts as a barrier to doing it.
